Mobile health initiative's goal: Cut hospital costs, readmissions

Community EMS, a Southfield-based ambulance and consulting company, has begun pilot testing a mobile health initiative to use paramedics and telemedicine to assess the health of chronic disease patients who develop non-emergency health problems.

The pilot is viewed as a way to reduce costly hospital readmissions and unnecessary visits to emergency departments.

"When a patient is discharged from a hospital to home or long-term care facility and they have a chronic illness, many things can trigger a patient being sent back to the hospital," said Greg Beauchemin, CEO of Community EMS.

Metro Detroit Rehab Warming Unit

As temperatures once again drop into the negatives with arctic wind chills, Community EMS will be providing its rehab warming unit to assist those in need.  This unit will provide food, clothing, and transportation to temporary warming centers for the night.

Despite the cold, CEMS hearts are warm!

As the weather got colder and the snow came down more, CEMS assembled a team of individuals that went out and helped the homeless or those less fortunate who were without heat, water, or food. Coats, hats, gloves, scarves, socks, shoes, pants, blankets, etc. were also handed out. As a result of the publicity of the event, community sup-port and donations of food, clothing, and even firewood poured in from several volunteers.
